Obtain A Repeat Prescription?
Please give 48 hours' notice.
To avoid error and confusion, WE CANNOT ACCEPT REQUESTS ON THE TELEPHONE.
It is safer for you if you either make your request by letter, or, preferably, by using our special
repeat prescription print-out note attached to your prescription which details all your medication. If you
notice any error we would be grateful if you would let the receptionist know.
If you want your prescription to be sent to you in the post, please enclose a stamped, addressed
envelope. Some local pharmacies operate a collection service. If you would like your prescription to be
collected by one of them, please tick the appropriate name on the repeat prescription note.
From time to time, you may receive with your prescription a request to see a doctor or the nurse. This is
because most medications require regular reviews to ensure their greatest benefit. No further prescription
will be issued until you have seen the doctor.
THIS FORM IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE
METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
